Christian Care Communities is seeking a full-time MDS Coordinator RN for our NEW Christian Health Center West located on Magazine Street in downtown Louisville, KY (40203).

PART II: POSITION SUMMARY
The MDS Coordinator RN is primarily responsible for the timely and accurate completion of the Resident Assessment Instrument (RAI) including the Minimum Data Set (MDS), assisting in identifying the problems and potential care planning issues, coordinating completion of the Care Area Assessments (CAAs) and coordinating the interdisciplinary staff involvement in assessment and care planning while maintaining compliance with all applicable laws, regulatory and organizational standards.
PART III: POSITION REQUIREMENTS
Required Education: Bachelor’s degree in nursing from accredited college or university
Required Experience: 2 years’ in long term care with working knowledge of all long term care regulatory requirements; MDS 3.0, Resident Assessment Instrument (RAI) and Care Area Assessments (CAAs) process
Required License and/or Certification: Licensed as registered nurse in Commonwealth of Kentucky
Skills, Abilities and Knowledge
1. Excellent communication skills to include:
a. Ability to speak and read English
b. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
c. Ability to write routine reports and correspondence
d. Ability to speak effectively before groups of employees, organizations and others
e. Ability to communicate effectively with all levels of staff
2. Working knowledge of various software programs such as Microsoft Office products including Word and Excel
PART IV: ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
The incumbent performs job responsibilities without posing a significant risk of substantial harm to the health or safety of him/her or others while maintaining compliance with all policies and procedures of Christian Care Communities and within the parameters of the Organization’s mission, vision, and values.
1. Promotes the utilization of the Resident Assessment Instrument (RAI) to provide a structured, standardized approach for identification of a resident’s strengths and needs which can then be addressed in an individualized plan of care.
2. Completes assessment process within regulatory guidelines and facility policy and procedures:
a. Completes Minimum Data Set (MDS) timely and accurately
b. Completes Care Area Assessments (CAAs) ensuring CAAs are comprehensive and completed on time
c. Enters data into computer
d. Transmits data timely
e. Attaches all validating data
3. Develops comprehensive care plan based on resident assessment
a. Completes individualized care plan within time frames
b. Includes all necessary Triggers in care plan
c. Monitors care plans for updates related to new and discontinued treatments
d. Coordinates care plan team
4. Sets schedule for RAI process
a. Utilizes therapy and other treatments to set the correct ARD date to maximize reimbursement
b. Participates in RUGS huddles to ensure correct data and maximizing of reimbursement
c. Leads care plan team and ensures all disciplines are in compliance with timeframes
d. Reviews case mix reports for accuracy and works with the state surveyors if there are problems
e. Analyzes the RUG category before sealing
5. Educates certified nurse aides about correct coding and documentation of Activities of Daily Living (ADL) and other MDS items for accurate care tracker entry
6. Communicates with the administrator, director of nursing, and unit managers regarding MDS and other issues as necessary
7. Keeps current with changes to the RAI and regulations
8. Initiates and participates in quality assurance activities to monitor Quality Indicators (QIs) and to carry out quality improvement activities.
9. Conducts training and monitors all staff involved in the MDS process to assure facility wide compliance.
